Mri May Help Doctors Differentiate Causes Of Memory Loss

Can a CT or MRI scan detect dementia? – Dr Simon Farmer

UCLA Health

Using a software program, UCLA Health researchers were able to measure the volume of different regions of the brain, identifying areas where shrinkage may have occurred.

A UCLA-led study has found that MRI scans can help doctors distinguish whether a persons memory loss is being caused by Alzheimers disease or by traumatic brain injury.

The study, which also involved researchers at Washington University in St. Louis, is important because it could help prevent doctors from misdiagnosing Alzheimers disease a diagnosis that can be devastating for patients and their families, and can prevent them from receiving appropriate treatment. 30831-7/abstract” rel=”nofollow”> A 2016 study by researchers affiliated with the University of Toronto found that up to 21 percent of older adults with dementia may be misdiagnosed with Alzheimers.)

The current study, published in the Journal of Alzheimers disease, involved 40 patients whose average age was just under 68 and who were being treated by UCLA neurologists. All of the patients had suffered traumatic brain injury and later developed memory problems.

Dr. Cyrus Raji, the studys corresponding author and an assistant professor of radiology at Washington University, said one of the benefits of the approach is that it doesnt require specialized equipment beyond an MRI machine and the software the researchers used so it could potentially be performed at many medical centers.

Can Vascular Dementia Get Worse

Vascular dementia will generally get worse, although the speed and pattern of this decline vary. Stroke-related dementia often progresses in a stepped way, with long periods when symptoms are stable and periods when symptoms rapidly get worse. This is because each additional stroke causes further damage to the brain.

Physical Exam & Patient History

Because the first symptoms of CJD are sometimes constitutional, identifying the actual date of onset may require some probing. An assessment of the patients ability to complete activities of daily living helps quantify disease progression. Ask about any family history of neurological or psychiatric disease . Medical and travel history may help assess the risk of acquired CJD.

Mri Scans Can Be Inconclusive

Sometimes, MRI scans are inconclusive and more tests may be required. These tests include the positron emission tomography or PET test and the single-photon emission computed tomography, which provide images of brain activity based on blood flow or oxygen consumption.

The tests can help narrow down a diagnosis by revealing deficits common in Alzheimers that are distinct from other dementias. Unfortunately, these scans cannot identify the disease with certainty. They cannot reveal the microscopic changes in brain tissue, something that characterizes Alzheimers. The good news is that brain scan technology is continuously evolving and soon there will be more definite scan to detect Alzheimers.

In short, no blood test, brain scan or physical exam can definitely diagnose Alzheimers. The situation is further complicated by the fact that there are many conditions that can produce symptoms resembling those of early Alzheimers.

An MRI scan will definitely help to eliminate other conditions that can cause similar symptoms as Alzheimers but it cannot be used to give a conclusive diagnosis of Alzheimers. A more detailed evaluation is required for that.

How Is Ftd Diagnosed

Can A CT Scan Show Dementia (Is MRI Better?)

Assessments

Blood tests and a full physical examination are important to rule out other possible causes of symptoms.

A specialist normally an old age psychiatrist or neurologist may think a person has FTD after talking to them and to someone who knows them well. The specialist will take a detailed history of the persons symptoms and ask questions to understand the persons behaviour and abilities better.

Standard tests of mental abilities, which mostly focus on memory loss, can be less helpful in diagnosing FTD. More specialised tests of social awareness or behaviour may be needed.

Scans

CT and MRI scans are used to see what parts of the brain are most damaged. They can also rule out other possible causes of a persons symptoms, such as a stroke or tumour. If further tests are needed, more specialised brain scans will be carried out, such as PET and SPECT to measure the persons brain activity.

These scans are useful as they may find lower activity in the frontal and/or temporal lobes before a CT or MRI scan can find changes to the brain tissue of these lobes.

Further tests may include a lumbar puncture, which involves collecting and examining liquid from inside the spine and is carried out mainly in younger people.

Genetic testing

A specialist may recommend that a person with FTD symptoms has a genetic test. This can show if the persons condition is caused by a specific faulty gene.

When Doctors Prescribe Brain Scans

Your physician might suggest that you get a brain scan to identify underlying problems causing mental conditions or affecting your general wellbeing.

Typically they are used to detect tumors, strokes, as well as other problems THAT CAN spark dementia that may appear on brain scans.

The cortex of the brain appears overly wrinkled and it has gyri which are separated by sulci .

Individuals with cortical atrophy experience the progressive loss of neurons which in turn causes the thinning of the ridges and the sulci to grow wider.

When brain cells continue dying, the brains fluid-filled cavities expand and occupy the available space.

In turn, they become LARGER than normal.

These structural changes within the brain are also aspects that BRAIN SCANS CAN IDENTIFY.

Future Directions In Diagnosis Research

The clinical role of MRI in dementia

Considerable research effort is being put into the development of better tools for accurate and early diagnosis. Research continues to provide new insights that in the future may promote early detection and improved diagnosis of dementia, including:

  • Better dementia assessment tests that are suitable for people from diverse educational, social, linguistic and cultural backgrounds.
  • New computerised cognitive assessment tests which can improve the delivery of the test and simplify responses.
  • Improved screening tools to allow dementia to be more effectively identified and diagnosed by GPs.
  • The development of blood and spinal fluid tests to measure Alzheimers related protein levels and determine the risk of Alzheimers disease.
  • The use of sophisticated brain imaging techniques and newly developed dyes to directly view abnormal Alzheimers protein deposits in the brain, yielding specific tests for Alzheimers disease.

What Are The Benefits Of Early Diagnosis

Early planning and assistanceEarly diagnosis enables a person with dementia and their family to receive help in understanding and adjusting to the diagnosis and to prepare for the future in an appropriate way. This might include making legal and financial arrangements, changes to living arrangements, and finding out about aids and services that will enhance quality of life for people with dementia and their family and friends. Early diagnosis can allow the individual to have an active role in decision making and planning for the future while families can educate themselves about the disease and learn effective ways of interacting with the person with dementia.

Checking concernsChanges in memory and thinking ability can be very worrying. Symptoms of dementia can be caused by several different diseases and conditions, some of which are treatable and reversible, including infections, depression, medication side-effects or nutritional deficiencies. The sooner the cause of dementia symptoms is identified, the sooner treatment can begin. Asking a doctor to check any symptoms and to identify the cause of symptoms can bring relief to people and their families.

Negative Mri Scan With Symptoms Of Stage 4 Dementia

would MRI scans may show brain shrinkage , while SPECT and PET scans show areas of loss of function. The pattern of these changes helps to diagnose the cause of the dementia. A scan that shows no unexpected changes in the brain does not rule out conditions such as Alzheimer’s disease because in the early stages of the disease the changes can be difficult to distinguish from normal ageing. The consultant may discuss their findings or may refer the person back to the GP for this information. In some cases the consultant may wish to see the person again after some months to observe any changes before making a diagnosis.Tips: Getting the most from a consultationWhether you are attending the appointment for yourself or for someone you are supporting, it may be useful to do the following:Write down any questions or worrying signs beforehand to ask the GP or specialist. It can be difficult to remember everything you want to say during a consultation. Write down any important points the doctor makes during the consultation.Ask a doctor, or any other professional, to explain words or phrases if you do not understand.Ask a doctor to write down any medical terms, particularly if English is not your first language.

Mri Scans As Accurate As Lumbar Punctures In Identifying Alzheimers Or Ftld

They found that by studying the structural brain patterns the density of gray matter on the MRI scans, their predictions were 75% accurate when confirming diagnosis with people who had pathology-confirmed diagnoses and those with biomarker levels retrieved from lumbar punctures this shows that the new MRI use is as accurate as lumbar puncture methods.

McMillan said:

Developing a new method for diagnosis is important because potential treatments target the underlying abnormal proteins, so we need to know which disease to treat. This could be used as a screening method and any borderline cases could follow up with the lumbar puncture or PET scan.

This method would also be helpful in clinical trials where it may be important to monitor these biomarkers repeatedly over time to determine whether a treatment was working, and it would be much less invasive than repeated lumbar punctures.

Additional Tests To Treat And Manage Dementia

White matter lesions on magnetic resonance imaging in ...

Once an individual is diagnosed with dementia, the next step that follows is helping them understand how the condition will affect them and how to manage it.

Several other medical assessments exist to help physicians understand how the condition affects a specific person. And also help families, as well as caregivers, figure out the best course of treatment for the individual.

Neuropsychological Testing

Neuropsychologists and psychologists who have specialized training can also prescribe neuropsychological tests to detect dementia.

The process involves WRITTEN and ORAL tests that can take several hours to complete.

They use these methods to assess the cognitive functions of the person suspected to have dementia.

It helps them figure out if certain areas are impaired.

The tests assess aspects like vision-motor, memory, comprehension, reasoning, coordination, and writing abilities.

Physicians may administer additional tests to find out if the person in question is SUFFERING from mood problems or dementia.

Functional Assessments

Dementia is a cognitive disorder that affects the afflicted persons daily functioning in different regards.

Objective assessments can establish what a person is STILL ABLE to do versus what they can no longer do in light of the condition.

Family members are asked to fillquestionnaires that provide details about the persons daily life in terms of the activities they are able to perform.

Blood Tests To Check For Other Conditions

Your GP will arrange for blood tests to help exclude other causes of symptoms that can be confused with dementia.

In most cases, these blood tests will check:

  • liver function
  • haemoglobin A1c
  • vitamin B12 and folate levels

If your doctor thinks you may have an infection, they may also ask you to do a urine test or other investigations.

How Is Alzheimer’s Disease Diagnosed And Evaluated

No single test can determine whether a person has Alzheimer’s disease. A diagnosis is made by determining the presence of certain symptoms and ruling out other causes of dementia. This involves a careful medical evaluation, including a thorough medical history, mental status testing, a physical and neurological exam, blood tests and brain imaging exams, including:

Structural Signs Of Dementia

Structural imaging of the brain consists of computed tomography and the popularly known magnetic resonance imaging scans. This kind of imaging focuses on the morphology as well as the structural details of the brain’s composition. It is a very physical kind of scan, searching for solid, visible signs of degeneration or abnormalities.

Degenerative dementia causes a number of visible physical signs in the brain in some patients, but is not always easy to detect. CT scans can usually observe some atrophy of the brain’s medial temporal lobe, but the CT scans’ lack of sensitivity can occasionally be problematic. MRI scans, of much higher resolution, can capture atrophy of the hippocampus in nearly 90 percent of all cases of Alzheimer’s disease.

Why Doctors Consider Mri To Detect Dementia

Can a brain scan indicate that someone is susceptible to dementia or Alzheimer’s?

Medical experts will advise on the use of MRI when they suspect that a person has dementia.

MRI uses focused radio waves and magnetic fields to detect the presence of hydrogen atoms in tissues in the human body.

MRI scans also reveal the brains anatomic structure with 3D imaging allowing doctors to get a clear view of the current state of the organ.

This way, the doctor is able to rule out other health problems like hydrocephalus, hemorrhage, stroke, and tumors that can mimic dementia.

With these scans, physicians can also detect loss of brain mass that relates to different types of dementia.

fMRI records blood flow changes that are linked to the activities of the brain. This may help physicians differentiate dementia types.

Verywellhealth.com also suggests that MRI scans can at times identify reversible cognitive decline.

In such a case, a doctor will recommend appropriate treatment that will reverse this decline and restore cognitive functioning.

Ruling Out Other Conditions

Once patients present to their GP, their clinician must rule out any other conditions, such as vitamin B12 or B1 deficiency, anaemia or depression. You dont want to start treatment for dementia when the real cause for the neurological presentation is something that is easily reversed, says Dr Hayo. But short appointment lengths make it difficult for GPs to get the full picture about a patients health especially if the patient has been sent by their family. GPs are only as good as information theyre given, says Dr Hayo. Some people with vascular dementia experience symptoms before 65, and these can be harder to pick up.By nature, GPs tend to look for the most common condition based on a patients age, she explains. Thats why it takes twice as long for under 65s to get a diagnosis its kind of ruled out and another diagnosis is given depression, anxiety, stress, relationship disturbance. over 75, it can be more expected due to increasing age.GPs must also assess the level of a patients cognitive impairment, for example by using the six-item cognitive impairment test . Although tests such as this can have their limitations3, they give an indication of a patients memory and cognitive function.4If the patients cognitive test suggests they need to be assessed further, GPs can then refer patients for diagnostic scans, such as CT or MRI scan of the head.

Testing For Vascular Dementia

However, timely access to these scans depends where you live in the country, according to Dr Hayo. This corresponds with Dr Taylors experience. isnt too bad at the moment where I work, but I have known long waits in the past, she says.Where I work, the referral form asks us to consider doing a head CT and we can order this directly, but thats not the case for GPs everywhere, she says.Dr Hayo says that CT scans are the cheapest option but not the most useful for diagnosing dementia. MRI scans are more appropriate because they can pick up subtle changes in the brain. Many people cant access MRI in some areas there is a four-month wait, she says. And the scan reading ability of the person who reports on the scan can vary as well as not everyone is skilled in identifying the early changes of dementia.Patients are usually then referred to a memory assessment clinic if they require specialist assessment. But this is another area where waiting times can slow down diagnosis.

What Is The Main Cause Of Alzheimers Disease

We wont be wrong to say that the Alzheimers disease comes with age, which is why it is the main cause. However, just like all types of dementia, the actual cause related to Alzheimers is the death of brain cells.

Alzheimers disease is a neurodegenerative one, which means that there is progressive brain cell death which happens over time. In a nutshell, the brain tissue of the person suffering from it starts losing its nerve cells and connections, resulting in this disease . Read more about what causes Alzheimers.
