Multiple skin changes the alterations in the barrier function of the stratum corneum , and proteases, pH variations, and decreased activity of sebaceous and sweat glands in the elderly are related to xerosis and chronic pruritus . The SC is a barrier to prevent transepidermal water loss and provides protection from external factors. The SC constantly undergoes cellular turnover, and as it ages, the normal process of desquamation can be altered, leading to the appearance of dry skin .

Stage : Moderately Severe Decline

Stage 5 of Global Deterioration Scale of Alzheimer’s disease, termed “moderately severe decline,” is determined when the individual is no longer able to do routine tasks without some assistance. Typical problems of the previous stages become worse added to these problems are the patient’s inability to remember to do common daily tasks , for example, the person:

  • May have difficulty remembering common information
  • Can no longer do routine tasks such as making a meal without some assistance
  • May become disoriented to time or place
  • Has a noticeable decline in personal hygiene habits, for example, brushing the teeth and/or bathroom hygiene

How To Help Prevent Itching And Scratching

If your loved oneâs scratching or picking is related to Alzheimerâs disease, it may help to give them something to do with their hands. Give them a safe object, like a washcloth, busy blanket, or a small, soft squeeze ball. You can also give them something theyâve used as part of their job or a favorite hobby. It may help to trim nails short, cover the area with a gauze bandage, or have them wear long-sleeved shirts that are difficult to roll up or unbutton. They may need to wear gloves, especially at night.

Also help your loved one with hygiene. They should bathe every other day with unscented, gentle soap. Use lotion, creams, or ointments after a bath or shower and as needed to keep skin hydrated. Keep them away from things theyâre allergic to.

If they scratch and pick repeatedly, prescription medications may help. Talk with their doctor for advice.

Emollients should be suggested as first-line therapy for localized pruritus, patients with chronic kidney disease, and xerosis. Dry skin is caused by changes in the composition of epidermal lipids and increased transepidermal water loss. As the cutaneous barrier function is impaired, hyperkeratosis, erythema, and itching episodes occur . Moisturizers of mixed physiological skin lipids similar to physiological skin lipids are used to hydrate the stratum corneum, restore the barrier function, and relieve itching . Emollients can contain supplementary ingredients such as urea, polidocanol, menthol, or palmitoylethanolamide with anti-itch properties to target multiple components of the itch pathway . Yet, the fragrances and preservatives in moisturizers can cause the allergic contact dermatitis in some patients who have already itchy dermatitis hence, the repeat open application test is useful before use. Moreover, since the recovery of the skin barrier function against irritation such as surfactant or alkaline soap is slow for the elderly, a light shower that does not cause severe irritation with a cleanser with mild surfactant is suitable. Xerotic eczema might be worse with frequent visits and stay longin hot places, like saunas.

  • Distress or agitation this may be because the person is confused about where they are, who they are with or what they are meant to be doing.

Intense scrotum itching, especially at night, could be a sign that you have pubic lice or scabies. These tiny lice and mites can infect the area around your groin and cause a rash and sores along with the itching.

According to the journal BMJ, scabies causes a severe itch and the scabies mites often affect the male genital area. Even after successfully treating scabies, the itch in the scrotum and genitals can still persist for up to 6 weeks.16

Dr. John Cunha on MedicineNet says that pubic lice are often transmitted by sexual contact. They live in the coarse pubic hair around the genitals and can cause itching around the scrotum. You may also notice the lice crawling or eggs in your genital area.17

To get rid of pubic lice or scabies mites from around your genitals, you can try neem oil. One study into the acaricidal effect of neem oil found that it effectively killed off scabies mites.18

All you need to do is apply neem oil to your genital area once a day. Leave it on for a while then shower. This is because neem oil has a pungent smell. Repeat the neem oil application daily until you no longer have any lice or mites that cause itchy scrotum and pubic area.

When To Seek Medical Advice

The discomfort caused by a rash can be distressing for someone suffering from dementia and you may need to keep a close eye on them to prevent them from scratching too much and damaging their skin.

Psoriasis Of The Scrotum

Psoriasis is an inflammatory skin condition that can affect the scrotum and cause patches of itchy skin. Most doctors agree that people who suffer from psoriasis in general, are at higher risk of the skin disease affecting their genitals.

According to the journal Health and Quality of Life Outcomes, psoriasis commonly affects the groin area. This can cause itchy, scaly skin on the scrotum and can itch chronically. There may also be white lesions on the patches of itchy skin. Usually, the only way to treat psoriasis is to manage the itch.19
